We use prepositions of place to describe position.

The mouth is between the nose and the chin.
under
The mouth is under the nose.
The chin is under the mouth.
next to
The ear is next to the eye.

Doctor and patient.

Patient: No, my ankle is fine, but my shoulder hurts.
